Completely overkill. No judge is looking at hosting disputes for that amount. Small claims or chargeback is the route.
What actually works: the provider's own terms of service. Screenshot the TOS at purchase, not just the sales page. They change them. I've caught Contabo updating their fair use policy mid-contract. The old TOS saved a client.

I run everything off a Pentium III in my attic. No hosting disputes if you don't use hosting.
More seriously: the answer to "what do you use" is "whatever the provider's payment processor will accept as evidence." PayPal wants different things than Amex than your bank. I just ask myself "will this convince a bored clerk in 6 months?"
