One question: you say the domain was expired 11 months. The previous owner maybe is dead or sick? This explain why they dont change the email. Is sad but possible. Maybe you can find them and help.

Dark thought but plausible. 11 months expired plus 4 years ownership suggests someone who let it lapse, not someone who planned to sell. Could be anything: business failure, health, forgot to renew.
Historical WHOIS is redacted now. You will not find them through normal channels. The registrar has verified contact info but will not share it.
Let it go. Your duty is to stop receiving the data, not to reunite it with the owner. You have done that.
Following. Similar situation on OVH auction domain last year but it was just newsletters. Upgraded to medical and I would have panicked too.
Did anyone mention the pharmacy notifications might be auto-refill? Those systems do not stop until someone actively cancels. You could be getting them for years.
This is why the 550 reject is correct. Bounce messages get ignored by automated systems. Hard reject at SMTP time triggers list hygiene in well-run systems. Poorly run systems will retry forever but that is their problem, not yours.
Lisbon perspective: self-hosting email is already hard. Adding previous owner cleanup makes it harder. You are handling this correctly.

Maybe the issue is not distance but RackNerd's Amsterdam routing. I have heard complaints about their upstream there.

Not the issue I care about but since you asked: 12ms is fine. My old setup was 2ms to a local provider. The extra 10ms is the cost of routing through RackNerd's filtering. Acceptable tradeoff.
The drift-mx-01 hop is actually in Amsterdam per their looking glass. My traceroute was simplified. Full path goes through Copenhagen. 12ms total is reasonable.
Can we not derail into latency optimization. The medical email problem is resolved.
