Simple question. I'll start: remote hands at a OVHcloud facility in London once reseated what they described as a "loose" SFP in our core router. It was a 100G LR4 carrying a BGP session with a full table to our upstream. They didn't static discharge, didn't check polarity, and managed to bend two pins in the QSFP28 cage. Took 6 hours to get replacement hardware shipped. The RMA note said "customer-induced damage" until we sent the facility CCTV.
